                As parent or guardian of the student, I hereby confirm that my child has read and agreed to the contract below.
Agreement to this contract indicates a genuine intent to participate fully in all aspects of the activity in a manner that fully challenges me, is helpful to others and protects the environment. I will not smoke, drink alcohol, or use non-medically prescribed drugs while on the activity. I will respect the safety regulations of the activity. I understand that SMUS’s school rules apply to this activity in its entirety.                 As a parent or guardian of the student, and having reviewed the material provided by SMUS about the activity, hereby:
a) Consent to the student participating in the activity.
b) Recognize and understand the inherent risks associated with the activity including but not limited to the hazards of vehicle travel, wilderness travel, water travel, snow travel, accident/illness in places without nearby medical facilities, weather and other circumstances beyond the control of St. Michaels University School employees, related parties and agents.
c) Authorize the activity leaders to consent to medical treatment for the student by any qualified medical practitioner as may be necessary.
d) Acknowledge that I have read and 
understand the specific risks associated with the activity that may be described in the trip's description, and may include, but is not limited to: risks associated with international travel, such as illness, air travel, unfamiliar social and political environments, as well as other risks that may include adverse weather conditions, winds, wildlife, the use of hiking equipment, swimming, slippery trails, the use of camp stoves, campfires and travel within and outside the activity area. Consequences of these risks may result in physical or emotional injury, paralysis, drowning, death or damage to property or third parties.
e) Understand that even though there are a variety of measures and practices in place to reduce the frequency and severity of injury/illness, residual risk remains. Accidents and injuries can still occur.
